| Community Service Report This form is used for students needing community service hours. You can complete your community service hour at any non-profit organization such as Parks & Recreation, YMCA, church or house of faith. You will need a minimum of 20 hours for your humanities requirement. On-site students will need to use the same form if needing to complete community service hours in order to replace suspensions. |

| PE Attendance Form PE Attendance Form. Students will be supervised by any staff or trainer who is not a relative and over the age of 21. You can attend a health club, YMCA, community center, sports center or sporting facility. Students will receive .5 credits for every 45 hours of activity. |
